Please call the office to make an appointment in advance. If you are unable to keep your appointment, please call us as far in advance as possible so we may use that time to see another patient in need of care. We make a sincere effort to adhere to our appointment schedule and appreciate your patience if we are late because of emergencies.
We make every effort to decrease the cost of your medical care. Therefore, we request payment arrangements for all office services at the time they are rendered, unless prior arrangements have been made. We accept cash, check, American Express, MasterCard, Visa, and Discover for your convenience. We will bill the patient’s insurance company; however, payment is the patient’s responsibility.
If you would like to request assistance with paying your bill, please print out the financial assistance form. Return the completed form to the patient accounts department, Columbus Community Hospital, 4600 38th St., Columbus, NE 68601. If you have any questions regarding these programs, please contact the patient accounts department at 402-562-3159.
Please request all prescription refills through your pharmacy 5-7 days prior to running out. Special requests and authorizations should be made during our regular office hours, 8 a.m.-5 p.m. Monday-Friday, when our full records are available.
